Physical Therapist, Home Health | Portland Metro - Full-Time Make a meaningful impact where patients need you most. Assured Home Health, part of the Optum family of businesses, is seeking a Physical Therapist to provide one-on-one care to patients in their homes throughout Indianapolis.

• $10,000 sign-on bonus for full-time employment
• Monday through Friday schedule
• $125K to $145K salary based on 40-hour work week

As a Home Health Physical Therapist, you'll evaluate patients in their homes, develop individualized plans of care and provide treatment focused on improving mobility, function and independence.

You'll collaborate with patients, families, physicians and the broader care team while managing your visits and documenting patient progress.

• Current Physical Therapy license in the state of practice
• Current CPR certification
• Current driver's license and vehicle insurance, with access to dependable transportation

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $100,000 to $150,000 salary based on full-time employment. We comply with all minimum wage laws as applicable.
