OverviewOptum Home & Community Care, part of United Health Group, is building an integrated care model that addresses physical, mental, and social needs. The senior community care team provides care to patients in nursing homes and assisted living facilities, offering a seamless health journey.

Position Highlights- Work in assisted living and community‑based settings focusing on high‑risk patients.
- Manage 4–6 patient visits per day with extensive training and a focus on outcomes, not volume.
- Interdisciplinary collaboration for acute medical situations, chronic disease management, and sub‑acute rehabilitation.
- Conduct comprehensive assessments, advanced illness conversations, and advanced care planning.
- Document accurately in compliance with ICD‑10, CMS regulations, and EMR requirements.
- Coordinate care transitions across care settings and implement personalized care plans.
- Function independently with minimal supervision.

- For Nurse Practitioners:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Active, unrestricted NP license in the state of residence.
- Board certified by the American Academy of Nurse Practitioners or the American Nurses Credentialing Center in one of the following:
Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, Adult‑Gerontology Acute Care Nurse Practitioner, Adult‑Gerontology Primary Care Nurse Practitioner.
- For Physician Assistants:
- Graduate of an accredited Physician Assistant degree program.
- Active, unrestricted PA license in the state of residence.
- Board certified by the National Commission on Certification of Physician Assistants.
- DEA prescriptive authority or ability to obtain post‑hire (unless prohibited in state).
- Ability to obtain a collaborative practice agreement if applicable.
- Physical requirements:
lift up to 30 lbs, navigate stairs, and perform physical assessments. - Reliable transportation.
- New graduates encouraged.
- 1+ years’ clinical experience in senior living, geriatric medicine, or home care.
- Experience with complex behavioral, social, or functional patient needs.
- Understanding of geriatrics, chronic illness, advanced illness, and end‑of‑life discussions.
- Proficient computer skills and EMR documentation.
