Nurse Practitioner / Physician Assistant - Relocation Assistance (Azza Health Care Agency)

We are seeking a licensed Nurse Practitioner or Physician Assistant to lead skin and wound care teams in a fast‑paced, patient‑focused environment.

• Lead multidisciplinary wound care teams through education and collaboration.
• Travel weekly within assigned regions for wound rounds and house calls.
• Provide high‑quality wound assessment, treatment, and minor procedures, documenting progress in our EMR.
• Develop individualized treatment plans aligned with clinical guidelines.
• Collaborate virtually with medical specialists including physicians and surgeons.

Requirements: Active licensure, DEA license, reliable transportation, 1‑2 years wound care experience preferred, strong communication skills, and dependability.
