Optum is seeking a Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Multnomah, OR

Position Highlights

In Optum's Senior Living Program, people are our priority. We focus on assisted living and community-based settings, blending palliative care with internal medicine. Clinicians provide care to high‑risk patients within an interdisciplinary team. Because we are not fee-for-service, our focus is on quality outcomes, not volume. With an expectation of approximately 4‑6 patient visits per day, you’ll have the time to invest in your patients' well‑being and build meaningful relationships.

Primary

Responsibilities
  • Conduct comprehensive assessments.
  • Manage patients through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
  • Manage patients during short-term sub-acute rehabilitation episodes.
  • Ensure accurate and complete documentation, including ICD‑10 coding.
  • Communicate and collaborate with the interdisciplinary care team.
  • Conduct advanced illness and advanced care planning conversations.
  • Provide counseling and education to patients and caregivers.
  • Document findings comprehensively and in compliance with CMS regulations.
  • Engage facility staff, families, and clinicians to implement patient care plans.
  • Coordinate care transitions across care settings.
  • Review orders and interventions for appropriateness, adapting care plans to patient needs and wishes.
  • Function independently with minimal supervision.

Required Qualifications – Nurse Practitioner
  • Active and unrestricted NP license in the state of residence, or ability to obtain by start date.
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Board certified by the American Academy of Nurse Practitioners or the American Nurses Credentialing Center in one of the following:
    Family, Adult, Gerontology, Adult‑Gerontology Acute Care, Adult‑Gerontology Primary Care, or Gerontology.
Required Qualifications – Physician Assistant
  • Active and unrestricted PA license in the state of residence, or ability to obtain by start date.
  • Graduate of an accredited Physician Assistant degree program.
  • Board certified by the National Commission on Certification of Physician Assistants (NCCPA).
