Description:
Weekend work is required every other weekend. Acute care experience is necessary. First-timers may be accepted with clinical approval. Call is not required for this position. Candidates may need to float between similar units based on departmental needs. Local candidates are accepted. All time-off requests should be included upfront during application. Familiarity with Epic charting system is required. Candidates should have experience with HMOs, IPAs, advance directives, and protective services processes.
Experience in case management roles is preferred, particularly recent experience. The role requires knowledge of patient advocacy, discharge planning, and care coordination. Candidates must have flexibility in their scheduling and be prepared to work on holidays. References are needed covering relevant work history. The position may involve patient interaction across various age groups. Candidates should be prepared for a dynamic work environment involving teamwork and interdisciplinary support.
