RN - Case Manager

Job Opportunity

We are seeking a dedicated and experienced professional to join our team in Eureka, CA. The role involves working day shifts, specifically a 5x8 hour schedule from 08:00 AM to 04:30 PM. The successful candidate will have acute care experience and will be responsible for case management.

• Manage patient admissions and discharges.
• Coordinate care and ensure effective communication with healthcare teams.
• Assess eligibility for benefits and assist with prior authorizations.
• Implement discharge planning and follow up on case management needs.
• Maintain compliance with HIPAA guidelines and The Joint Commission standards.
• Handle appeals and denials related to patient care.
• Provide support for various patient populations, including adolescents, adults, and geriatrics.

• Minimum of 2 years of acute care experience in case management.
• Strong understanding of admission criteria and benefits eligibility.
• Experience with HMOs, IPAs, advance directives, and protective services processes.
• Proficiency in the Epic charting system.
• Ability to float between similar units as needed based on departmental requirements.
• Availability to work every other weekend and commit to working 3 out of 5 major holidays.
