We are seeking a dedicated Case Manager to join our healthcare team in Eureka, CA. This role requires acute care experience and is designed for candidates who have a passion for providing quality patient care and support.
Shift Information- Shift Type:
Day (5x8 Hour Schedule, 08:00 - 16:30) - Weekend Requirements:
Required to work every other weekend - Call Duty:
Call is not required - Floating:
Candidate will float between like units based on departmental needs
- Experience:
Minimum of 2 years of acute care experience required - First-timers:
Candidates accepted with clinical approval - Charting System:
Experience with Epic is required - Licensure:
Valid Compact license must be in hand at submission
- Conduct comprehensive assessments and develop patient care plans
- Coordinate care across the healthcare continuum
- Ensure compliance with CMS guidelines and HIPAA regulations
- Manage discharge planning and facilitate necessary support services
- Assist patients with admissions, benefits eligibility, and prior authorizations
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Experience with HMOs and IPAs
- Familiarity with advance directives and protective services processes
- Knowledge of DRGs (Diagnosis Related Groups) and Core Measures
- Ability to manage complex cases including pediatrics and geriatrics
