Full Time, Salary Range: $75,000.00 - $85,000.00 Salary/year, Job Shift: Day, Redwood Coast PACE - a Program of All-inclusive Care for the Elderly - is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults. At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible.
Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health.
The schedule for this position is Monday through Friday, 8 am to 5 pm, with occasional on-call support. This is a great opportunity for a nurse who is interested in leaving bed-side care and moving into management.
JOB SUMMARYLeads and supports the Home Care team, including in the areas of advocacy, education, coordination, documentation, and providing participant care. Directly supervises the in-home Care Partner team. Works in the capacity of a Licensed Vocational Nurse within the Home Care team.
ESSENTIAL DUTIES & RESPONSIBILITIES- Assists the Home Care Manager in recruiting, training, supervising, and evaluating employees.
- Develops and implements in-service training programs for employees.
- Maintains a safe work environment and ensures compliance with safety practices.
- Assists with department communication related to relevant program issues.
- Attends team meetings, participant conferences, in-service trainings, etc., as applicable.
- Participates in the process of teaching, training, and mentoring student interns and volunteers working within the program area.
- Attends meetings with outside agencies, caregivers, and families. Networks with other agencies on behalf of program and participants.
- Provides various internal and external training regarding medications, medical conditions, safety, general health education etc.
- Communicates with the physicians and caregivers of participants regarding health concerns and recommendations.
- Assists participants in obtaining medical services from other health service providers.
- Assists in scheduling agency transportation as needed.
- Assists with creating and implementing operational policies and procedures.
- Orders Durable Medical Equipment (DME) for participants, as needed.
- Oversees the facilitation and coordination of Care Partner scheduling. Conducts scheduling duties, as needed.
- Maintains individual participant attendance, skilled nursing care records, medication administration records, and other medical data as necessary to document medical nursing care.
- Under the direction of the Home Care Manager, files written and verbal Incident Reports as required by the program. Notifies families and caregivers of incidents.
- Monitors general health status of participants.
- Monitors participant nutritional status and refers to Registered Dietitian when appropriate.
- Administers treatments in accordance with physician orders.
- Monitors, administers, and records prescribed medications.
- Assists with personal care needs of participants, meals, and other general care duties.
- Provides various nursing services as directed by the Home Care Manager and Medical Director.
- Conducts Home Care Safety Assessments and reports back to the Interdisciplinary Team.
- Ensures all assessments are completed and documented in a timely manner.
- Conducts home safety visits as assigned, ensuring completeness of the enrollment process for Redwood Coast PACE.
- Participates in on-call coverage in conjunction with the Home Care team.
- Acts in a manner consistent with agency values.
- Performs other duties as assigned.
EDUCATION &/or EXPERIENCE
- High school diploma or GED equivalent and completion of a vocational, or practical nursing program approved by the California Board of Vocational Nursing and Psychiatric Technicians (BVNPT) required.
- Minimum of one (1) year experience working with a frail or elderly population required.
- Minimum of one (1) year experience in a supervisory role required. Additional supervisory experience preferred.
- Previous experience working as a Licensed Vocational Nurse and/or in a home-care setting preferred.
- Current California BVNPT Vocational Nurse Licensure required.
- Current CPR and First Aid certification within six (6) months of hire.
