Visit Nurse - RN

Position Overview The Visit Nurse provides high-quality, patient-centered, direct nursing care to individuals facing life-limiting illnesses. This role involves making scheduled and unscheduled visits to patients in their homes or residential facilities, ensuring comfort, dignity, and support for both patients and their families.

Essential Duties and Responsibilities Conduct in-home visits to assess patient condition, manage symptoms, and provide emotional support.

Administer medications and treatments as prescribed, including pain and symptom management.

Collaborate with interdisciplinary team members including case manager RNs, physicians, social workers, chaplains, and aides.

Educate patients and families on disease progression, care plans, and available resources.

Document all care provided and communicate updates to the hospice team.

Respond to urgent patient needs and provide crisis intervention when necessary.

Ensure compliance with hospice policies, procedures, and regulatory standards.

Required Education/Credentials:

Graduate of an accredited nursing program.

Current California Registered Nurse license.

Required Experience:

Minimum of one (1) year of experience as a professional nurse within the previous three (3) years or equivalent experience in a BSN program.

Schedule Requirements:

Must be able to work two (2) shifts per month, including weekends.

Preferred Qualifications Experience in hospice or home health.

Experience working effectively with an interdisciplinary team.

CHPN certification.
