Family Practice - OB Physician

Advent Health Well 65+ is seeking an experienced, Board Certified Family or Internal Medicine to join our team and provide personalized care to patients 65-years of age and older. This practice exclusively focuses on Medicare Advantage patients.

At work, you will experience:

• More Time with Patients:
  Up to 40 minutes with patients to provide comprehensive, personalized care
• Smaller Patient Panel. Max patient panel of 700.
• Lower Daily Patient Census. 14-16 patients per day.
• Clinic Support Team. Includes RN Case Management, 1.5 Health Coaches (MA s) per provider, Social Worker, Pharmacy, Care Coordinators, Welcome Coordinator, HCC Coding Support, and Administrator.

Education and Experience Required:

• Completion of a US-based Internal or Family Medicine Residency
• Experience in a Value-Based Care practice

Licensure, Certification, and Registration:

• Current, valid Florida medical license (or ability to obtain)
• Board Certification in Internal or Family Medicine by ABMS or AOA
