- Responsible for individualized case planning, discharge planning, and monitoring progress.
- Responsible for case management duties to include planning and implementing all facets of programming and associated documentation.
- Prepares, organizes, schedules, and implements psycho educational groups and interventions.
- Monitors patient compliance with established goals and objectives; updates case plans as needed.
Preference will be given to qualified applicants who are currently a certified Social Worker.
KNOWLEDGE- Knowledge of human behavior, motivation, and interpersonal behavior.
- Knowledge of the varied techniques of programming/education.
- Knowledge of social work principles and practices.
- Knowledge of interviewing and counseling techniques.
- Knowledge of current social, economic, and health problems and community resources.
- Education: Bachelor's Degree (typically in Social Services)
- Experience: 0- 2 years of progressive work experience (typically in Casework)
- OR
- Education & Experience Substitution: 3- 5 years of progressive work experience (typically in Casework)
- Certificates, Licenses, Registrations: None
- Physical ability to lift, twist, bend, pull, and push up to 50 lbs.
- FLSA:
Non-Exempt - Pre-employment drug and alcohol testing will be conducted on safety-sensitive positions. Drug testing will be completed on any position if reasonable suspicion exists.
- A background check, including the taking of fingerprints, will be completed through the Wyoming Department of Criminal Investigation, the Department of Family Services, and the Federal Bureau of Investigation for every successful applicant.
