Palliative Care provides patient-centered medical care to those with serious illnesses. As a Palliative Care provider, you will integrate your advanced clinical expertise with empathy, guiding patients and their families through challenging health journeys. The Advanced Practice Provider will help manage symptoms, provide education, coordinate transitions of care, and establish individualized goals of care over the course of a patient's illness.

This is a full-time position working 80 hours every 2 weeks. Palliative Care completes consults in the inpatient and outpatient setting. This position will be focused on outpatient care.
Position requires licensure in the state of Indiana, with certification by the ANCC/AANP as a Certified Nurse Practitioner (acute care or family practice) or NCCPA as a Certified Physician Assistant.
Fully implemented EMR (Epic), integrated for inpatient and outpatient.
CME/PTO hours.
