ResponsibilitiesAbout the Role
During visits that can last up to one hour, you will gain unmatched insights into a member’s overall health by reviewing their medical history, medication adherence, and social environment. The Matrix Comprehensive Health Assessment (CHA) allows you to improve quality of care and close multiple care gaps in a single visit. Your responsibilities will include:
- Conducting adult and geriatric assessments, including medical history, diagnosis, treatment, health education, and referrals.
- Administering and ordering point-of-care and lab-based testing as needed.
- Collaborating with physicians, case managers, social workers, and family members.
- Providing services at members' homes or Skilled Nursing Facilities.
- Flexibility: Schedule visits based on YOUR availability, with assessments conducted Sunday – Saturday from 8 am – 7 pm.
- Competitive Pay: Earn based on the number of assessments completed, with potential monthly earnings ranging from $1,800 – $11,500 .

Skills & Experience That Shine
- Master’s Degree or equivalent experience with NP licensure.
- Current NP licensure in your state, including prescription authority.
- AANP, AACN, or ANCC board certification in relevant specialties.
- Current BLS, ACLS, or CPR certification.
- 1 year of NP experience preferred; new grads are encouraged to apply!
- Valid state driver's license and proof of adequate automobile insurance.
- Strong computer skills and familiarity with health/medical record software.
- Excellent verbal and written communication skills.
