Sage Group Health Care Staffing is seeking RN Home Health for multiple travel / contract positions in Evansville, IN. Accepting both local and traveler candidates.

Pay Rate:
Range $2000 to $2080 Weekly Gross Pay based on a 40 hour work week (Includes Non-taxable travel/housing and M&I stipend for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri with additional shifts on weekends if the required weekly productivitypoints are not met. The Nurse can also choose which 5 days of the week they would like to work before the offer, for example Tuesday-Saturday
Position Requirements- Registered Nurse: 2years
- Required - Recent/Current Home Health Nursing
Experience:
1 year within the last 3 years – Required - OASIS experience: 1 year– Required
- Home Care Home Base(HCHB) charting experience – Required
- Must have valid
Driver’s License and Personal Auto Insurance - Must be Covid Vaccinatedor provide medical or religious exemption
- ASN or an equivalent– Required
- BSN or an equivalent– Highly preferred
- RN License for Stateof Indiana or Compact
- Required - BLS Certification from AHA or American Red Cross
- Required
- Provide and coordinate care for assigned patients in the comfort of their homes.
- Develop and implement nursing care plans, including Start of Care and admissions as assigned.
- Support patient recovery and ability to stay at home, enabling them to spend quality time with their loved ones and do the activities they enjoy.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Home health nursing care and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
- Ensure patients’status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting for all patient visits within 24 hours.

Gibson, Pike, Dubois, Posey, Vanderburgh, Warrick, Spencer, and Perry
