- Evaluates physical and functional status, as well as cognitive, psychological, and perceptual ability to determine the patient’s capabilities to function (safely)
- Documents all evaluations, treatment, and coordination of care contacts via appropriate facility policies
- Demonstrates an ability to treat a wide range of diagnoses and be knowledgeable in current therapy treatments, goals, and contraindications for treatment
- Provides education and counseling to patient/family/clients/providers regarding treatments and goals, potential safety risks and/or other issues within scope of practice
- Independently manages own case load to maximize effectiveness of overall care plan
- Complies with all applicable Federal, State, and local laws, regulations, and requirements, as well as facility/agency specific policies and procedures.
- Supports special projects and other designated activities as assigned in support of TAI’s mission and core values
- Works collaboratively with care team members to include but not limited to, MD, PA, RN, Behavioral Psychologist, Health Coach, Nutritionist, Care Management, etc.
- Responsible to managing and driving MSK care within Care Team
- Active Physical Therapist license
- 4+ years of clinical experience
- Commitment to top quality patient care.
- Eagerness to develop your skills and support your team members as they develop their skills.
- OCS and/or NAIOMT certification preferred
Physical Therapist base compensation is $98,000-$108,000+ depending on experience and fit. Relocation/sign-on bonus available!
