Substance Abuse Counselor; SUDP

We provide persons in need of mental health support with wraparound services which include counseling, therapy, EMDR, medication management and employment services. Our staff is comprised of a tightly knit team of local therapists, case managers and psychiatric prescribers who enjoy a laid back, casual work environment.

• Provide information, counseling, and referral services in an individual and/or group setting for assigned clients and families to contribute to their rehabilitation and to the physical and mental health of Sunrise Community Substance Use Disorder clients. Provide on-site coverage and crisis intervention when necessary.
• Provide chemical dependency therapy and case management services to clients.
• Assess clients' presenting problems, formulate individualized treatment plans, apply appropriate treatment modalities, assess, monitor and document clients' response to clinical interventions. Evaluate treatment effectiveness and revise treatment plans regularly.
• When appropriate, provide chemical dependency services in group settings and individual treatment.
• For clients whose symptoms may be managed through appropriate use of psychotropic medications, make referrals to the Sunrise Psychiatric & Professional Services (SPPS) prescriber for medication management consultation.
