Optum Home & Community Care, part of United Health Group, is seeking an Advanced Practice Clinician (NP/PA) to join the House Calls team in Snohomish County, WA. You will perform in-home health assessments for Medicare Advantage and other plan members, educating patients and coordinating with PCPs to close care gaps.
You will conduct exams, screenings and health education, with no on-call, weekends or holidays, and flexible hours between 8 a.m. and 8 p.m.
