** We are seeking a dedicated Home Health Registered Nurse to provide compassionate care to patients in their homes.
** Responsibilities:*
* • Deliver quality nursing care in home settings.

• Conduct assessments and create care plans.

• Use Oasis and HCHB for records.

• Perform Start of Care, routine visits, and recertifications.

• Manage approximately 5 patient visits daily.
