The Case Manager Liaison Nurse will provide and coordinate nursing care for members in collaboration with the healthcare team, focusing on medication management and optimal patient health outcomes. This role is ideal for nurses who excel in a collaborative environment and have a strong background in case management.

• Ensure patients referred to case management meet established criteria.
• Assess patients for physical, mental, financial, and psychosocial status.
• Develop patient-centered case management plans with specific goals and time frames.
