Overview

Responsible for providing high quality nursing care to patients in their own homes on the Isle of Wight, as part of the Integrated Community Nursing Service. The role involves working flexibly across three locality teams and collaborating with partners including Earl Mountbatten Hospice, Urgent Community Response, Allied Health Professionals, GPs, Practice Nurses, Specialist Nurses, mainland hospitals and voluntary or statutory agencies.

Responsibilities
  • Assess, plan and deliver person‑centred care in community settings.
  • Provide evidence‑based assessment, treatment and continuity of care for patients with complex needs.
  • Travel across the Isle of Wight using own vehicle with business insurance; hold a valid UK driving licence.
  • Work independently in a busy community environment, ensuring patient safety and adherence to manual handling and infection control principles.
  • Collaborate with multidisciplinary partners and maintain clear communication with patients, families and colleagues.
  • Maintain accurate and timely documentation for the Trust’s electronic patient record system.
  • Participate in ongoing professional development and contribute to quality improvement initiatives.
Person Specification
  • Qualification: Registered Nurse (current NMC registration).
  • Experience: Experience in community nursing preferable; clinical experience in urinary catheterisation, syringe driver management, wound and diabetes care desirable but not essential.
  • Driving: Full UK driving licence and access to a vehicle with business insurance; must be a clean licence, capable of lone working.

    Good interpersonal and communication skills, organisational ability, motivation and self‑management, awareness of personal safety, flexible and professional role model, key behavioural competencies.
  • Knowledge: Current issues in community nursing, principles of manual handling and blood‑borne disease prevention.
  • Other: Must undertake Disclosure and Barring Service check and comply with the Rehabilitation of Offenders Act (Exceptions Order) 1975.
