Job Overview
Provide pharmaceutical care services to patients of Exeter Hospital, including oversight, monitoring, preparation, dispensing, and proper documentation of medication therapy. Offer drug information, oversee dispensing systems, and ensure compliance with federal, state, and local laws and regulations. Follow physician orders, review medication orders, and compound and dispense medications and other pharmaceuticals.
Responsibilities- Review and monitor patient medication therapy and orders written by appropriate caregivers for safety and efficacy, considering specific patient conditions and health status; advise the patient‑care team of any issues or possible enhancements and document interventions.
- Consult with physicians and other health practitioners on selection, dosages, interactions, and side effects of medications, including renal dosing, monitoring culture and sensitivity reports, lab value appropriateness, medication level monitoring, and adverse drug events.
- Prepare, label, and dispense medications accurately and efficiently in accordance with physicians’ prescriptions; ensure compliance with federal, state, and local laws and regulations, including hospital policies related to dispensing all medications, drugs, narcotics, and maintaining required records.
- Maintain appropriate dispensing records and oversee systems that charge or credit medications and treatments.
- Ensure proper documentation and record‑keeping for all medications, with particular attention to controlled substances.
- Oversee and monitor all aspects of the medication process; collect data and present reports and recommendations regarding issues with the medication systems.
- Prepare sterile solutions using USP 797 guidelines and non‑sterile solutions using USP 795 guidelines.
- Lead technical staff to ensure accurate and timely delivery of medications to patient‑care areas; orient, train, and mentor new staff as appropriate.
- Doctor of Pharmacy (Pharm‑D) or Bachelor of Science in Pharmacy with advanced training
- 1–3 years experience
- Pharmacist licensed in NH
