LVN​/LPN – LTC​/SNF

Job Summary:

The LVN/LPN is responsible for providing nursing care to residents in a long-term care or skilled nursing facility setting. This role involves implementing patient care plans, administering medications, and assisting with daily activities to ensure the well-being of residents.

Responsibilities:

• Deliver direct patient care based on established nursing standards and facility policies.
• Administer medications and treatments accurately and in a timely manner.
• Document patient assessments, interventions, and responses in electronic health records.
• Collaborate with interdisciplinary team members to develop and update care plans.
• Monitor residents' condition, report changes, and escalate concerns as needed.
• Ensure compliance with health and safety regulations and infection control protocols.

Qualifications:

• License as an LVN or LPN in the state of New Hampshire.
• Proven experience working in a long-term care or skilled nursing environment.
• Strong assessment and patient care skills.
• Ability to follow detailed instructions and maintain accurate documentation.
• Excellent communication and teamwork abilities.
• Knowledge of state and federal healthcare regulations and infection prevention protocols.
