LVN​/LPN – LTC​/SNF

Job Summary:

Providing skilled nursing care in a long-term care and skilled nursing facility setting, the LVN/LPN will deliver quality patient care under the supervision of registered nurses and physicians. This role involves maintaining a safe, comfortable, and compassionate environment for residents.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Monitor patient vital signs and assess residents’ conditions regularly.
• Document patient care activities comprehensively and promptly.
• Collaborate with healthcare team members to ensure proper resident care.
• Respond to emergency situations and provide basic life support if necessary.
• Follow infection control protocols and maintain a clean, safe environment.

Qualifications:

• Valid LVN or LPN license in the state of New Hampshire.
• Minimum of 1-year experience in long-term care or skilled nursing facilities.
• Proven ability to assess and prioritize patient needs efficiently.
• Strong communication skills and ability to work as part of a team.
• Completion of adult CPR and medication administration training.
• Ability to adhere to health and safety protocols and policies.
