LVN​/LPN – LTC​/SNF

Job Summary:

This licensed practical nurse (LPN) role involves providing quality nursing care in a long-term care or skilled nursing facility. The position requires delivering patient-centered assistance and collaborating with healthcare teams to ensure optimal resident outcomes.

Responsibilities:

• Administer medication and treatments accurately according to physician orders.
• Monitor residents’ health status and document changes in condition.
• Assist with daily living activities, including bathing, dressing, and feeding residents.
• Collaborate with interdisciplinary teams to develop and update care plans.
• Maintain compliance with state and federal regulations, including documentation and safety protocols.

Qualifications:

• Current license as an LPN or LVN in the state of New Hampshire.
• Proven experience working in a long-term care or skilled nursing environment.
• Knowledge of infection control procedures and safety regulations.
• Strong communication and interpersonal skills with residents and team members.
• Ability to effectively document resident care and health information.
