Position Summary

The Case Manager is responsible for group therapy, documentation requirements, and additional ancillary services. The Case Manager is familiar with 12-Step Recovery programs and has knowledge of documenting group notes in an electronic medical record system. Works as a team member and works collaboratively with the Primary Counselor and Clinical Director in providing quality treatment services and client care.

Responsibilities
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient-centered.
  • Handle case assignments, draft service plans, review case progress and determine case closure.
  • Communicate patient needs to a variety of care team members and follow up accordingly.
  • Manage discharge plans upon completion of treatment.
  • Attend clinical and administrative meetings for inter-departmental sharing of information.
  • Work collaboratively with patients, families, therapist, and referral sources to ensure high quality care.
  • Facilitate multiple care aspects (case coordination, information sharing, etc.).
  • Maintain the patient's comprehensive clinical record through detailed documentation.
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
  • Promote quality and cost-effective interventions and outcomes.
  • Record case information, complete accurately all necessary forms and produce statistical reports.
  • Help navigate legal issues that patients are currently facing, up to including appearance in court hearings.
  • Adhere to professional standards as outlined by protocols, rules, and regulations.
  • Act as the patient's advocate regarding insurance coverage and financial assistance.
  • Continuously monitor and evaluate each client’s progression through their case management plan and develop corrective action revisions to the plan as needed.
  • Arrange abeyance with social services, health, and governmental agencies.
  • Perform other duties as assigned by the supervisor, consistent with the position and in compliance with agency policies and procedures.
Requirements

Skills, Knowledge, and Abilities

  • Acute understanding of counseling and other treatment procedures for alcoholism and drug addiction-related problems.
  • Knowledge of regulations concerning alcoholism and drug addiction treatment.
  • Knowledge of alcoholism and drug addiction treatment procedures for such illnesses.
  • Ability to manage tasks and strong interpersonal skills.
  • Demonstrates strong problem-solving and analytical skills.
  • Able to work independently.
  • Communicates effectively (both written and verbal) with customers and colleagues.
  • Organized, detail-oriented, multi-tasker, adaptable, positive.
  • Proficient computer skills, including Microsoft Office Suite.

Preferred Experience / Minimum Qualifications

  • Bachelor's Degree in human services field.
  • Certified Drug and Alcohol Counselor recognized by the state.
  • Minimum of 1+ years experience as a group facilitator in Substance Abuse and Mental Health settings preferred.
  • Knowledgeable of state licensing agency guidelines.
  • Must be familiar with 12 Step Recovery programs.
  • Knowledge of Electronic Medical Records.

Physical Requirements
  • Office/Sedentary Requirements – Incumbent must be able to perform the essential functions of the job. Work is performed primarily in an office environment. Typically requires the ability to sit for extended periods of time (66%+ each day), ability to hear the telephone, ability to enter data on a computer and may also require the ability to lift up to 10 pounds.
  • Exposure to clients infected with Hepatitis B, HIV, TB, or other infectious diseases.
