We are looking for an enthusiastic and patient-focused Young Persons Social Prescriber to join our friendly multidisciplinary team 3 days a week.

This is a varied and rewarding role, supporting children and young people to improve their health and wellbeing by connecting them to community services, activities and support networks. You will work closely with the wider primary care team, schools, families, and external agencies to provide holistic, person-centred care, helping young people address social, emotional, and practical needs that impact their health.

Main duties of the job

Develop, implement and support health promotion and wellbeing programmes for children and young people

Provide personalised support to young people, linking them to community services, activities and appropriate support networks

Assess, develop and review personalised care plans, ensuring accurate record keeping and effective use of clinical systems and coding

Identify and support young people at risk of poor health outcomes, prioritising need and referring to appropriate services where required

Recognise and respond to mental health and safeguarding concerns, escalating in line with local policy and procedures

Work collaboratively with GPs, practice staff, schools, families and voluntary sector organisations to deliver holistic care

Liaise with external agencies to ensure coordinated support for vulnerable young people, contributing to safeguarding processes

Support young people to build confidence, resilience and independence in managing their own health and wellbeing

Maintain up to date knowledge of local community services to enable effective signposting and social prescribing

Contribute to team meetings, shared learning and service development, adapting practice in line with local and national priorities

St Thomas Medical Group is the second largest practice in Devon and a forward thinking, innovative organisation. We are looking for an enthusiastic, skilled and proactive Young Persons Social Prescriber to help further develop and diversify our workforce.

We operate across three sites in Exeter St Thomas, Exwick and the University of Exeter providing care to a large and diverse population.

This is an exciting opportunity to join a friendly, supportive and well established multidisciplinary team. Our workforce includes 11 Partners, 17 Salaried GPs, 22 Nurses, 3 ANPs, 3 Paramedics, 2 Adult Social Prescribers and First Contact Physiotherapists, all working collaboratively to deliver high quality patient care

Job responsibilities

Main Duties and Responsibilities

Provide non-clinical, person centred support to children and young people, focusing on wellbeing, emotional health and social needs

Assess individual needs and co-produce personalised care and support plans

Support young people to access appropriate services, including community groups, youth services, education, mental health and voluntary sector provision

Develop and maintain a comprehensive knowledge of local services and resources

Identify young people at risk of poor outcomes, offering early intervention and enabling access to appropriate support

Recognise safeguarding concerns and escape in line with practice and local safeguarding procedures

Work collaboratively with GPs, nurses, social prescribers, schools, families and partner agencies to provide holistic care

Build strong working relationships with external services to ensure coordinated support

Maintain accurate, contemporaneous records using Systm One, ensuring appropriate coding and data entry

Collect and report outcome data to support service evaluation and development

Contribute to reducing GP workload by supporting non-medical needs

Promote self care, independence and resilience in young people

Participate in team meetings, supervision and ongoing service development

Person Specification Experience
  • Experience working with children and young people or within community, health, education or voluntary sector settings
  • Strong interpersonal and communication skills, with the ability to engage young people effectively
  • Good IT skills, including experience of using electronic clinical systems or willingness to learn Systm One
  • Ability to work independently and as part of a team
  • Understanding of safeguarding and confidentiality
  • Organised, proactive and adaptable approach to work
  • Previous experience in a social prescribing or personalised care role
  • Knowledge of local community services and resources
  • Experience using Systm One
  • Understanding of mental health and wellbeing support for young people
Qualifications
  • Empathetic and non judgemental approach
  • Ability to build trust and rapport with young people
  • Strong organisational and time management skills
  • Confident using IT systems and maintaining accurate records
  • Commitment to improving health outcomes and reducing inequalities
