Position: Health Referral Facilitator Full Time
GLL is proud to have entered an exciting new chapter, having secured a 13-year leisure contract with Lewisham Borough Council from April 2026. This milestone brings a fantastic opportunity to build on the success of our Healthwise service and further expand our impact across the community. We are now looking to grow this well-established physical activity referral scheme by introducing two new specialist pathways - Cancer Rehabilitation and Better Balance (falls prevention) - enabling us to support more residents in improving their health, wellbeing, and quality of life.

GLL is looking for a Health Referral Facilitator to work across the sport and leisure facilities in Lewisham. The Health Referral Facilitator role is responsible for prescribing suitable physical activity and supporting behaviour change for Medically Referred clients.

The Health Referral Facilitator role will include setting up and undertaking group and individual training sessions along with general day to day operation of the GP Referral programme including referral processing, accurate record keeping and data processing. The Health Referral Facilitator is responsible to the Healthwise Coordinator / Health Manager.

What you’ll do:

To ensure the smooth day-to-day running of the Physical Activity Referral Scheme (PARS), maximising effectiveness of all available resources. Carry out health and lifestyle assessments with referral patients and develop safe and appropriate programmes of physical activity / exercise. Act as first point of contact for patients referred onto the scheme To provide individuals with evidence-based, relevant and culturally sensitive information regarding a healthy lifestyle.

To constructively interact with different people in a variety of areas within the operational team, wider health teams and GLL support services. As a leader within the leisure industry we can also offer scope to progress your career, and job variety like nowhere else. What you need: GP Referral qualification (Level
3) - Essential A recognised Fitness instructor or Personal training qualification (Level
3)

Experience of pre-screening / assessing patients for suitability to physical activity / exercise programme
