About the Role
As a Support Worker, you will play a key role in delivering high-quality, enabling care. You will help individuals achieve independence, manage risks, and maintain well-being through person-centered support.
Key Responsibilities- Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance.
- Activity planning: develop flexible, engaging activity plans that promote autonomy and personal growth.
- Health and lifestyle: support individuals in maintaining healthy lifestyles and managing their health needs.
- Communication: facilitate effective communication and maintain detailed, person-centered records.
- Medication management: oversee medication processes and support individuals in self-administration.
- Safety and environment: maintain a safe, clean living environment and adhere to health and safety protocols.
