We are excited to advertise a permanent Clinical Pharmacist position working across Exeter and Langdon, Dawlish,
localities. The Clinical Pharmacist will gain experience and provide
clinical input to a range of inpatient wards, including:
Adult acute mental health Psychiatric intensive care Rehabilitation Older peoples mental health (including dementia)
Secure services (including low and medium security) Eating disorders
Perinatal
Under the supervision of a Senior Clinical Pharmacist,
the post-holder will be supported to develop their experience and interest in
mental health, neurodiversity and learning disability services. The postholder
will have close clinical support via regular managerial and clinical
supervision with their line manager, access to an array of friendly,
experienced and supportive pharmacists and pharmacy technicians across the
Trust as well as a variety of networks including a regular pharmacy peer group.

*** Main duties of the job Working closely with the Rotational Pharmacists, Senior
Clinical Pharmacists, Clinical Pharmacy Technicians and Medicines Optimisation
Technicians, the postholder will provide expert clinical pharmacy service
across Langdon and Exeter inpatient wards.
The postholder will attend relevant multi-disciplinary
team meetings such as ward rounds, risk-review meetings and discharge planning
meetings to ensure that clinical experience and expertise is provided to
patients with complex medication regimes and critical medications. In a
targeted approach, the Clinical Pharmacist will review clinical information
from the electronic patient records as well as gathering the views and wishes
of the patient, carers and/or relatives to recommend and guide evidence-based and
person-centred treatment plans.
The potholder will also provide line management and
support the Rotational Pharmacists to develop and progress within their roles.
Clinical Pharmacists will also support wider ward-based action plans to address
areas of improvement relating to medicines management.
