Key Responsibilities- Client Support: Work directly with individuals experiencing homelessness to assess their needs and connect them to housing solutions in alignment with the Housing First model.
- Case Management: Develop personalised housing plans, coordinate resources, and provide ongoing support to ensure clients are able to maintain their housing successfully.
- Collaboration: Work closely with community partners, including mental health professionals, substance use teams, and healthcare providers, to ensure clients receive comprehensive support.
- Crisis Intervention: Provide immediate assistance and de‑escalation support to clients in crisis, ensuring their safety and well‑being.
- Documentation & Reporting: Maintain accurate and timely case notes, assessments, and other required documentation to track client progress and service delivery.
- Advocacy: Act as a champion for clients, advocating for their needs with service providers and ensuring they have access to the resources they deserve.
- Community Engagement: Support clients in accessing community‑based services, including job training, healthcare, and mental health services, to promote long‑term stability.
