Position

Housing First Case Coordinator (part‑time)

Key Responsibilities
  • Client Support:
    Work directly with individuals experiencing homelessness to assess their needs and connect them to housing solutions in alignment with the Housing First model.
  • Case Management:
    Develop personalised housing plans, coordinate resources, and provide ongoing support to ensure clients are able to maintain their housing successfully.
  • Collaboration:

    Work closely with community partners, including mental health professionals, substance use teams, and healthcare providers, to ensure clients receive comprehensive support.
  • Crisis Intervention:
    Provide immediate assistance and de‑escalation support to clients in crisis, ensuring their safety and well‑being.
  • Documentation & Reporting:
    Maintain accurate and timely case notes, assessments, and other required documentation to track client progress and service delivery.
  • Advocacy:
    Act as a champion for clients, advocating for their needs with service providers and ensuring they have access to the resources they deserve.
  • Community Engagement:
    Support clients in accessing community‑based services, including job training, healthcare and mental health services, to promote long‑term stability.

Requirements
  • Experience:

    Experience working with individuals who have experienced homelessness or related services. Lived experience with housing instability or homelessness is highly valued and considered equivalent to professional experience.
  • Skills:

    Strong interpersonal skills with the ability to build trust and rapport with diverse populations. Excellent communication, organisational, and problem‑solving abilities.
  • Knowledge:
    Familiarity with the Housing First model and trauma‑informed care practices.
  • Empathy & Compassion: A genuine desire to help people overcome barriers to stable housing and improve their quality of life.
  • Flexibility:
    Ability to work independently and as part of a team, manage multiple priorities, and adapt to the evolving needs of clients.
