Position: Housing First Case Coordinator (Salary: £25252.58 - £27120.13 per annum, Permanent, Full Time, Mon - Fri, 9am - 5pm)

Join our dynamic team as a Housing First Case Coordinator and help provide life-changing support to those who need it most. Key Responsibilities:

  • Client Support: Work directly with individuals experiencing homelessness to assess their needs and connect them to housing solutions in alignment with the Housing First model.
  • Case Management: Develop personalised housing plans, coordinate resources, and provide ongoing support to ensure clients are able to maintain their housing successfully.
  • Collaboration: Work closely with community partners, including mental health professionals, substance use teams, and healthcare providers, to ensure clients receive comprehensive support.
  • Crisis Intervention: Provide immediate assistance and de-escalation support to clients in crisis, ensuring their safety and well-being.
  • Documentation & Reporting: Maintain accurate and timely case notes, assessments, and other required documentation to track client progress and service delivery.
  • Advocacy: Act as a champion for clients, advocating for their needs with service providers and ensuring they have access to the resources they deserve.
  • Community Engagement: Support clients in accessing community-based services, including job training, healthcare, and mental health services, to promote long-term stability.
