Case Manager - Supportive Housing

Apply to join FACETS as a Case Manager in our Permanent Supportive Housing (PSH) program, which provides housing and case management for 68 individuals and families living in group homes, apartment units, and single‑family residences through programs sponsored by HUD and Fairfax County. The role serves as the primary case manager and service coordinator for participants in FACETS’ Permanent Supportive Housing (PSH) programs.

Typical responsibilities include supportive counseling, community living skills training, crisis interventions, and coordination among property managers, participants, and community partners.

FACETS is a non‑profit organization that helps parents, their children, and individuals who suffer the effects of poverty in Fairfax County. We meet emergency needs through wrap‑around services and community partnerships, ensuring safe and stable housing for those in need.

Duties and Responsibilities
  • Provides case management and program coordination for individuals in FACETS’ Permanent Supportive Housing (PSH) programs who are chronically homeless and medically vulnerable as identified through the VI‑SPDAT.
  • Coordinates participant selection into the programs, including meetings at local homeless shelters and other outreach activities.
  • Coordinates and manages volunteers and mentors assigned to each program participant.
  • Serves as an ongoing liaison between property managers, participants, and neighbors.
  • Provides mediation and advocacy with landlords on the participant’s behalf.
  • Creates and maintains consistent communication channels among tenants, landlords, referral sources, and collaborating agencies.
  • Coordinates services using a wrap‑around/HOST/team approach.
  • Travels and makes home visits, working an adjusted schedule including evenings and weekends as necessary.
  • Conducts monthly home inspections for permanent supportive housing participants.
  • Informally engages participants by providing emergency supports such as food, clothing, hygiene supplies, and similar items.
  • Provides case management, information, and referral to supportive services as needed, including medical, dental, mental health, substance abuse, social services, and employment services.
  • Provides transportation to medical and dental services and informs participants of other resources.
  • Documents all services identified and provided, referrals made, and transportation provided. Enters all data into HMIS.
  • Plans and coordinates life skills groups.
  • Manages crisis situations, assesses safety issues, and assists participants in accessing emergency services.
  • Assists in managing the FACETS budget for program expenditures.
  • Prepares proposals, reports, and statistics for submission to HUD under the supervision of the Senior Program Director.
  • Participates in program staff meetings and conferences, collaborates to meet FACETS goals.
  • Participates in and conducts in‑service training for staff development.
  • Supports FACETS’ mission‑oriented outcome evaluation and outcome management.
  • Represents FACETS in the community, aligning with our mission, vision, and code of ethics.
  • Performs other duties as assigned.
Required

Skills and Abilities
  • Bachelor’s Degree plus two years of professional social work/case management experience with homeless and/or at‑risk populations. MSW strongly preferred.
  • Strong familiarity with the chronically homeless medically vulnerable population as identified through the VI‑SPDAT.
  • Knowledge and belief in “Housing First” and “Rapid Re‑Housing” philosophy and strategies.
  • Ability to work in a fast‑paced environment and manage an intensive caseload with minimal supervision.
  • Knowledge of current social service, homeless, and housing issues and approaches to address them.
  • Ability to use HMIS to establish and maintain case records and facilitate data collection.
  • Strong communication skills—both oral and written.
  • Ability to schedule and manage workload to meet deadlines.
  • Valid driver’s license, reliable transportation, good driving record, and personal car insurance.
  • Must pass criminal background and Child Protective Services background checks.
  • Bi‑lingual (English/Spanish) a plus.
