About the Role

The Prevention Case Manager supports survivors in the community experiencing domestic and sexual violence, stalking, and/or human trafficking who are at imminent risk of homelessness. This position is responsible for providing time-limited direct services to clients by assessing client needs, connecting clients to appropriate services, and advocating to ensure the provision of comprehensive services which support survivors in increasing safety and achieving housing stability.

The Prevention Case Manager utilizes their knowledge of community resources to provide families with connection to legal services, case coordination, and other supportive services in the community as needed. Case management services are provided in a client-centered manner driven by the victim's needs and goals and respecting client self-determination.

How you will contribute
  • Perform in-depth assessments of clients' physical, mental health and social needs, life skills competency, employability, housing history and barriers, educational needs, financial management skills, strengths and needs, and other necessary areas, in conjunction with the client, to develop and achieve housing goals.
  • Administer flexible financial assistance to support client goals related to safety and stability.
  • Provide mobile advocacy services by meeting with assigned clients regularly in a safe location in the community.
  • Provide crisis intervention and develop individualized safety plans as needed; assist victims of violence in filing for protective orders and completing affidavits; accompany clients to court proceedings and various appointments (i.e. attorney meetings, protective order hearings, custody hearings, and housing interviews).
  • Provide housing search assistance to clients by identifying housing leads, assisting clients to apply for units, and accompanying clients on housing tours.
  • Establish relationships with local landlords to negotiate lease terms and advocate on behalf of clients.
  • Collaborate with clients to develop monthly budgets; provide money management education and connections to financial literacy programs as needed.
  • Document all contacts with clients in client file and VADATA as required; maintain complete client records in accordance with agency and best practice standards.
  • Provide domestic violence psychoeducation to clients utilizing tools such as The Power and Control Wheel and the Cycle of Violence.
  • Empower clients and their household members as they work toward their individual goals; complete ongoing reviews of client service plans.
  • Work with the Senior Case Manager to ensure that services are implemented in accordance with agency philosophy, best practices, and all applicable federal, state and local regulations.
  • Collaborate with community providers and allied professionals to ensure the delivery of comprehensive wrap‑around services.
