Position: Physician - Patient Aligned Care Team (PACT) - Fairfield

Overview

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program approval—award amount up to $200,000 and eligibility period one to five years—are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Former EDRP participants are ineligible to apply.

Qualifications

To qualify for this position you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physicians in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation.

  • United States Citizenship:
    Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
  • Degree of Doctor of Medicine or an equivalent degree
  • The degree must have been obtained from a school approved by the Department of Veterans Affairs for the year in which the course of study was completed
  • Current, full and unrestricted license to practice medicine or surgery in a state/territory/commonwealth of the United States or in the District of Columbia
  • Residency Training:
    Physicians must have completed residency training approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification
  • Approved residencies include: (1) ACGME; (2) AOA; (3) Other residencies followed by at least five years of verified practice in the United States
  • Residents currently enrolled in ACGME/AOA accredited residency programs may be appointed as "Physician Resident Providers" (PRPs) and must be fully licensed; PRPs may be appointed on an intermittent or fee-basis and are not considered independent practitioners
  • Additional requirements:

    Board certified or board eligible in Internal or Family Medicine (preferred); experience in Primary Care - Internal Medicine or Family Medicine

  • Physical requirements:

    Visual acuity, keen hearing, clear speech, and manual dexterity
  • Ability to perform long periods of walking, standing, stooping, sitting, bending, pulling, and pushing; transferring patients and objects may be required
  • Exposure to infected patients and contaminated materials; may require donning protective clothing
  • Occasional exposure to patients who are combative secondary to delirium, dementia, or psychiatric disorders
  • Must be mature, flexible, and able to work effectively in stressful situations; able to shift priorities based on patient needs
  • Annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment
Duties
  • The incumbent will be responsible for assessment and care of new and returning patients with medical problems (e.g., hypertension, diabetes, ischemic heart disease, COPD, depression, chronic pain)
  • Care may be delivered via face-to-face, telehealth, or video visits; may require travel to VA facilities or Community Based Outreach Clinics (CBOC)
  • Order diagnostics, consultations, medications, and other therapies; meet preventative health maintenance needs; complete VA computer documentation including clinical reminders and screenings in a timely manner
  • Telehealth and/or video visits may be conducted from VA-approved work sites
  • May support the Call Center
  • Major duties include evaluation (history and physical), diagnosis, consultation, and treatment of adult patients in the clinic environment
  • May perform diagnostic or therapeutic procedures as appropriate (e.g., joint injections, wound care, laceration repair, etc.) and care for patients via face-to-face or telemedicine modalities
  • Perform preliminary interpretation of ECG and radiographic studies
  • Supervise residents, fellows, and others in training
