Responsibilities- The Population Health Medical Social Worker (PHMSW) is responsible for providing social services to “high-risk” capitated populations utilizing a Population Health approach.
- Coordinate with inpatient/outpatient healthcare teams, payers, and outside agencies to link patients to appropriate services.
- Facilitate integration with community-based resources and collaborate with community partners to optimize patient health.
- Assist patients in navigating and accessing community resources.
- Document assessments and interventions in the patient’s electronic health record.
- Adhere to system policies, procedures, quality assurance guidelines, and organizational goals.
- Education:
Master of Social Work degree from a school accredited by the Council on Social Work Education. - Licensure/Certification: MSW; valid California driver’s license, access to an automobile, with automobile insurance meeting agency guidelines.
- Experience:
Minimum of two years in a health-related or community-based setting with familiarity of community resources. - Interpersonal
Skills:

Strong psychosocial assessment, communication (verbal, written, listening), and ability to work both independently and collaboratively. Able to work with diverse populations and provide emotional support to patients/families. Strong clinical judgment and decision-making, with ability to engage effectively with healthcare teams and community partners. Proficient computer skills.
$49.73 - $57.38 per hour. Based on years of experience.
