Home Health Aide – PRN – Solano
Must have CNA and HHA certifications.
The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient's needs. The home health aide is responsible for observing patients, reporting these observations, and documenting observations and care performed. The home health aide will be assigned by a Registered Nurse in a manner that promotes the quality, continuity, and safety of a patient's care.
EssentialJob Functions / Responsibilities
- Assist the patient with toileting activities.
- Plan and prepare nutritious meals.
- Assist in feeding the patient, if necessary.
- Take and record oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure.
- Assist in ambulation and exercise according to the plan of care.
- Perform range of motion and other simple procedures as an extended therapy service as ordered (with appropriate completed/demonstrated skills competency).
- Perform simple procedures as an extension of nursing services as ordered (with appropriate completed/demonstrated skills competency).
- Assist patients in the self‑administration of medication.
- Do the patient's laundry, as appropriate.
- Meet the safety needs of patients and use equipment safely and properly (footstools, side rails, etc.).
- Report on the patient's condition and significant changes to the assigned nurse.
- Adhere to the Organization's documentation and care procedures and standards of personal and professional conduct.
- Adhere to the company attendance and reliability guidelines.

Job Qualifications- Meets training requirements and successfully completed a training and competency evaluation program that meets 5484.80 (b) and (c); or
- A competency evaluation that meets the requirements of 5484.80 (c); or
- A nurse aide training and competency evaluation approved by the state as meeting the requirements of through S483.154 and is currently in good standing on the state nurse aide registry; or
- The requirements of a state licensure program that meets the provisions of 5484.80 (b) and (c).
- Shows proof of registration on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau.
- At least 18 years of age.
- Ability to read and follow written instructions and document care given.
- Self‑directing with the ability to work with little direct supervision.
- Empathy for the needs of the ill, injured, frail, and impaired.
- Maintain current CPR certification.
- Demonstrate tact, patience, and good personal hygiene.
- Possess a valid California Driver’s License with a vehicle that is insured in accordance with Organization requirements and is in good working order.
- Ability to sit occasionally for 1‑3 hours.
- Ability to frequently stand for 3‑6 hours.
- Ability to occasionally drive/operate foot control for 1‑3 hours.
- Ability to walk on level surface for 1‑3 hours.
- Ability to walk on unlevel surface for less than 1 hour.
- Ability to bend or climb for less than 1 hour.
- Ability to twist, kneel, crouch, and crawl for 1‑3 hours.
