Under the supervision of the MSSP Site Director/Supervising Care Manager, the Registered Nurse Care Manager (RNCM) functions as geriatric care manager for frail low‑income elders enrolled in the MSSP Medi‑Cal Waiver program. The RNCM evaluates the potential client as a total person and identifies the functional and health limitations that impede independent living. They recommend, provide, and coordinate a wide range of social and health services to assist MSSP participants in maintaining safe community living.
The MSSP Registered Nurse Care Manager works as a member of the Choice in Aging (CiA) team providing health and social services to frail elders and adults with disabilities to meet CiA’s mission to create opportunities where people can learn, grow and age independently with dignity and community.
- Screen applicants to MSSP using established criteria and screening forms to determine level of care appropriate for MSSP.
- Conduct initial health assessment – in participant’s home or via phone.
- Reassess the medical portion of the yearly reassessment in person or via phone.
- Reviews and completes initial MSSP contracts with participants (Application, Release of Information).
- Identify any/all medical‑related need placing the participant at risk for injury or placement.
- Bachelor’s Degree in Nursing (public health, health education, health administration, gerontology, or equivalent).
- 1‑2 years of clinical nursing experience, preferably with frail older adults or one year of case management experience.
- Hold a valid current license to practice nursing in the State of California, a public health certificate issued by the State of California is a plus.
- RNCM must have the ability to communicate in English in both written and verbal forms.
- Must have valid California Driver’s license, clean DMV record and automobile insurance.
- The RNCM must possess skills to manage care for vulnerable crisis‑prone participants with diverse medical and social problems.
- Strong computer literacy to work in electronic data management system and Excel.
- Current first aid/CPR certification required.
- Must pass Department of Justice fingerprint clearance.
Being fully vaccinated means that an individual is at least two weeks past their final dose of an authorized COVID‑19 vaccine regimen. Final candidates must be fully vaccinated as of their first day of employment.
