About the Role:The Advanced Practice Registered works with a team of professional social workers, clinicians, case managers and others to ensure integrated care for adults experiencing serious mental illness. As a member of the client care team, the APRN will establish therapeutic relationships in order to facilitate client progress with treatment goals. To be effective, the APRN will have the demonstrated ability to balance independent clinical decision making with appropriate consultation from providers.
This role includes providing psychiatric care and medication-assisted treatment (MAT) for clients with co-occurring mental health and substance use disorders.
- Complete clinical assessment evaluations and re-evaluations for assigned clients, and as needed.
- Develop individualized plan for treatment based on clinical assessment.
- Provide medication evaluation and management, medical consultation with ongoing monitoring of effects of medication use in coordination with primary care providers as necessary.
- Provide on-call coverage.
- Take a leadership role as a member of an interdisciplinary treatment team to ensure a coordinated and responsive treatment planning and evaluation process for assigned clients.
- Communicate changes in client treatment planning, medical needs and service needs to all staff involved in the provision of core services to clients.
- Ensure compliance with all policies, procedures and protocols related to the provision of psychiatric services to clients.
- Document clients’ medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, notes or outcomes in Electronic Medical Record within assigned timeline.
- Communicate vital operational policy and procedural announcements to Agency staff as necessary.
- Develop and promote contacts, working relationships and cooperative arrangements between the Agency and other organizations.

- Attend required program meetings and relevant training activities.
- Responsible for understanding and following all agency policies.
- Learn and follow all code of conduct policies and the agency’s code of ethics.
- Acquire knowledge and grasp all agency health and safety policies including participation in drills and attending all related training sessions as required.
- Provide psychiatric medication evaluation and management, including MAT medications, with ongoing monitoring of effects in coordination with primary care providers as necessary.
- Master’s Degree in Nursing along with the appropriate Nurse Practitioner certification required.
- Psych certification or candidate must be working towards psych certification.
- Unrestricted, current and complete State of Vermont APRN license required (or ability to attain upon employment).
- Experience providing psychiatric care and medication-assisted treatment (MAT) for adults with severe and persistent mental illness and co-occurring substance use disorders required.
- Experience working with adults with severe and persistent mental illness, substance use disorders, developmental disabilities, and co-occurring developmental disabilities and/or medical conditions required.
- Demonstrated current knowledge of psychiatric medications.
- Solid computer skills and familiarity with operating an Electronic Medical Record required.
- Ability to demonstrate compassion, sensitivity, and reasoning skills.
- Must have a valid driver’s license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.

- Competitive pay -$160,000 – $165,000 (compensation is typically dependent upon experience)
