Prestige Care of Fairfield is a leading provider of subacute rehabilitation and skilled nursing care in the community. We are dedicated to improving the quality of life for our residents by providing compassionate, evidence-based therapy services in a supportive and collaborative environment.

We are seeking a licensed Physical Therapist (PT) to join our in-house rehabilitation team. This role will focus on providing high-quality physical therapy services to residents in our subacute rehabilitation unit, helping them achieve their highest level of function and independence. This position offers flexible scheduling with full-time and per diem opportunities available.

• Evaluate, plan, and implement individualized physical therapy programs for residents.
• Provide therapeutic interventions, including mobility training, balance exercises, strengthening, and gait training.
• Monitor and document resident progress, updating care plans as needed.
• Collaborate with physicians, nursing staff, occupational therapy, and speech therapy to ensure coordinated care.
• Educate residents and families on exercises, adaptive techniques, and safe mobility.
• Participate in team meetings and quality improvement initiatives.
• Maintain compliance with federal, state, and facility regulations.

• Licensed Physical Therapist in the state of Iowa.
• Minimum of 1 year of experience in a skilled nursing or subacute rehabilitation setting preferred.
• Strong clinical reasoning, communication, and documentation skills.
• Ability to work independently and as part of a multidisciplinary team.
• CPR certification required or willingness to obtain.

• Full-Time:
  Monday–Friday, standard business hours (flexibility may be required).
• Per Diem:
  Flexible scheduling based on facility needs.
