Join us as a Nurse Practitioner/Physician Assistant Primary Care at Eventus Whole Health, where you will deliver integrated primary care to adults in skilled nursing and assisted living facilities.
Position SummaryEventus Whole Health provides an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including physicians, nurse practitioners, physician assistants, medical assistants, and in‑house support staff.
Essential Duties and Responsibilities- Provide primary‑care services per Eventus Whole Health site‑specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine in long‑term care settings.
- Review and sign medication administration records (MARs) requested by facilities; sign primary‑care orders at the facility or within 24 hours if faxed or emailed (phone orders, DME orders, death certificates, plans of care, PT/OT orders, hospice orders, etc.).
- Review and address lab results within 24 hours, manage narcotic prescriptions per Eventus Whole Health Narcotic Rx Policy, and provide sufficient administrative time during each visit.
- Perform on‑call triage duties according to the on‑call triage protocol; adhere to the latest negotiated triage schedule and policies.
- Complete documentation and submit billing records per published charting and bill‑sheet policies; provide the facility a hard copy of electronic documentation within 48 hours of service.
- Maintain open, positive communication with facility staff and administration; provide verbal or written instruction on medications and caregiver information; meet with the facility administrator or care coordinator before patient visits each day.
- Adhere to Eventus Whole Health standards, policies, procedures, and all applicable legal and regulatory rules; comply with professional ethical standards.
- Maintain patient confidentiality, including proper use of cell phones, emails, text messaging, charts, and EMR; comply with HIPAA rules and regulations.
- Consult with the clinical supervisor before refusing a resident, family, or facility request regarding resident care.
- Master’s degree in Nursing from an accredited institution and a current valid license to practice medicine (or nursing) in the state of residence; all required certifications must be current.
- Alternatively:
Graduation from a physician assistant program accredited by the National Commission on Accreditation of Allied Education Programs, passing the national certification exam, and holding a valid state license as a Physician Assistant. Specialization in geriatric, family, or psychiatry is desired.
