We are searching for a talented Advanced Practice Practitioner to join our Health at Home Hospice team.

Hours:
Full-time - 40 hours

8A - 6:30P 4 days/week

Shift: Day - no weekend, holiday or on-call (occasional on call to back up medical director - no scheduled on-call responsibility ongoing)

** Available to regular status employees who are scheduled to work a minimum of 24 hours.

  • Under direction of the Director of Supportive Care Services or designee, work in collaboration with a Physician, and other palliative care team members, in an active practice to deliver hospice care and related health care services to patients and families.
  • Perform a wide variety of duties including but not limited to: bedside patient/family evaluation, assisting with diagnosis, treatment and management of acute and chronic health conditions, educating/counseling patients, families, and/or caregivers as to preventative care, medical problems, etc and serving as a nursing resource for education and consultation to members of the interdisciplinary team.
  • Requires a Masters of Science Degree in Nursing.
  • Certification from a national accrediting body to practice as a Nurse Practitioner is required.
  • Current RN license in the Commonwealth of Massachusetts & Rhode Island is required.
  • Minimum of 5 years nursing experience with clinical nursing care in oncology, hospice, geriatrics and/or intensive care preferred.
  • A minimum of one-year Nurse Practitioner experience.
  • Certification in Hospice and Palliative Care required.
  • Must possess current CPR certification.
  • Certification in Hospice and Palliative Care required.
  • Current RN license in the Commonwealth of Massachusetts is required.
