Seeking a DPT or Physical Therapist to cover therapy services in Fairhope, AL, to provide treatment in a home setting.

Responsibilities (not limited to):

• Evaluates and treats patients in a home setting.
• Communicates with families, physicians, and other health care professions.
• Maintains daily thorough documentation of services in the EMR.
• Has own equipment including smart devices and blood pressure monitor.

Qualifications:

• Graduate of an accredited college or university with a Doctoral/Masters/Bachelors degree in Physical Therapy.
• Education and/or
  
  Experience:
  
  New graduate applicants welcome + Experienced Therapists especially hospice patients.
• Actively Licensed as a FL Physical Therapist.
