How you will make an impact

Vista Prairie at Goldfinch is seeking an Aspire Care Coordinator to join our compassionate team. As a dedicated Aspire Care Coordinator you will work within the framework of Vista Prairie Communities’ values of Caring, Respect, Collaboration, Innovation, and Stewardship. You will demonstrate commitment to helping residents find purpose and enhance their quality of life. This position involves leading, developing, and training care staff, as well as assisting with care staff schedules and monitoring service delivery to residents under the guidance of the Community Care Manager.

Your key accountabilities will include delivering and managing care through the Aspire Lifestyle Model to honor residents and enhance their quality of life. You will engage residents in living their life with purpose using the Aspire 5 pillars of wellness, including social, physical, intellectual, emotional, and spiritual. You will demonstrate stewardship and leadership by setting a positive example, fostering a supportive work environment, and boosting team morale.

You must also be dedicated to exceeding expectations to ensure seamless care for residents. As an Aspire Care Coordinator, you will also maintain a high standard of care and professionalism while effectively following the guidance of CCM and utilizing resources to meet the needs of residents, and much more. Together, we can make a meaningful impact on the lives of our residents.

Schedule: Full-Time, Evening shift, 2PM - 10PM, working weekends, and the call rotation.

What you will need
  • High school diploma or GED preferred.

  • Must be 18 years of age or older.

  • Training in health-related disciplines and/or relevant experience required

  • Supervisory and scheduling experience preferred.

  • Excellent communication skills, both written and verbal

  • Technology proficient
