Overview

Sub‑Acute, Long‑Term Care is a rapidly growing sector of healthcare offering flexible work‑life balance with no on‑call or weekend duty. We seek a full‑time (40 hrs/week) provider for our Pierpont Center in Fairmont, WV. Candidates may be a Nurse Practitioner or Physician Assistant, including new grads. Salary is $105,000–$120,000 per year.

Responsibilities

The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. The role collaborates on‑site with the Center Leadership Team to establish center‑specific goals, works with the Medical Director and/or attending physician for patient care, and provides primary and urgent care services to nursing center residents in partnership with other medical staff. The provider works collaboratively with Center Leadership to further Align Med Partners’ mission of delivering high‑quality, compassionate health care.

Qualifications

• Graduate of an accredited School of Nursing, licensed as a registered nurse, and certified as a Nurse Practitioner (Master’s in Nursing, concentration in Older Adult and/or Family NP). Other concentrations may be considered based on experience and business need.

• Physician Assistants must graduate from an ARC‑PA accredited program, pass the PA National Certifying Exam, hold PA state licensure, NCCPA certification, and an unrestricted professional license with DEA for each state of practice.

• Minimum of two years experience with adult or geriatric populations is preferred.

• APRNs in states permitting practice must hold an unrestricted professional license and DEA where required.
