Southwest General Home Health is seeking an Occupational Therapist I to provide comprehensive skilled OT in patients' homes under physician orders. You will educate patients and families, ensure safety, and maintain accurate documentation in line with Medicare and agency guidelines.
The role requires at least one year of OT experience, current state license, and the ability to collaborate with care teams while supervising COTAs and coordinating care transitions across the agency network.
