OverviewFind Your Passion and Purpose as a Hospice Care Consultant. Join a purpose-driven, collaborative culture at Accent Care where you will provide compassionate care to patients, families, and care partners. As a Hospice Care Consultant you will build relationships with physicians, nurses, and facility staff; educate referral sources; maintain accurate CRM documentation; and partner with clinical staff to ensure smooth transitions of care.
Key Responsibilities- Build meaningful relationships with physicians, nurses, and care facility staff through daily in-person visits and educational conversations.
- Collaborate closely with Admissions and Marketing teams to develop and carry out community outreach plans.
- Provide consistent education and compassionate guidance to referral sources.
- Maintain accurate and timely documentation in the CRM system while tracking outreach efforts.
- Partner with clinical staff to ensure each referral transitions smoothly into care, prioritizing patient comfort, dignity, and family support.
- Participate in trainings, plan thoughtful outreach strategies, and honor confidentiality in all aspects of work.
- Bachelor’s degree preferred, or 5+ years of experience in hospice, home health, healthcare sales, or marketing in lieu of a degree.
- Excellent written and verbal communication skills; strong ability to collaborate with a range of stakeholders and interdisciplinary teams.
- Skilled problem‑solving, understanding referral patterns, and navigating Medicare regulations and hospice reimbursement practices.
- Proficient with Microsoft Office Suite; available for after‑hours and weekend referrals as needed.
- Physically capable of frequent standing, walking, typing, and working on hard/rough surfaces; occasional lifting, kneeling, reaching, and exposure to chemicals, dust, extreme temperatures, and other conditions.
- Maintains constant visual, auditory, and verbal engagement; able to drive, adapt to varying indoor/outdoor settings, and complete all documentation with attention to compliance and detail.
