SAH Social Worker LICSW
Brown University Health – Saint Annes Hospital, Fall River, MA
Pay Range: $66,397.24 - $
Job Type: Full-time
Shift: Day (M-F 8:00am-5:00pm, 8 hours daily)

- Develop and execute patient discharge plans, coordinating required services upon discharge.
- Provide psycho‑social interventions and conduct ongoing reassessments for assigned patients throughout their hospital stay.
- Accept referrals from outpatient areas and refer to community agencies as necessary.
- Provide education/consultation to Nurse Managers and nursing staff to achieve quality patient outcomes.
- Complete psychiatric evaluations and placements for patients with mental health or substance abuse complaints presenting to the ED.
- Collaborate with multidisciplinary team to identify high‑risk medical/social management patients and develop individualized plans.
- Coordinate discharge follow‑up activities; discuss with families the financial ramifications of decisions and provide guidance.
- Deliver individualized psychotherapy, supportive counseling, and group therapy as needed.
- Act as a patient advocate, documenting assessments, plans and discharge information in medical records promptly.
- Communicate with Skilled Nursing Facilities, community health agencies, physicians and other staff in a timely manner.
- Maintain current knowledge of regulatory requirements and ensure service excellence standards are upheld.
- Participate in quality improvement initiatives and utilization management committee meetings.
- Evaluate psychiatric patients in the ED and make disposition recommendations.
- Perform case management and credentialing for independent practitioner status when applicable.
- Minimum of 2 years of case management and managed care experience.
- At least 2 years of experience in a clinical setting.
- Experience developing and facilitating patient discharges from an acute care setting.
- Excellent assessment, diagnostic and crisis intervention skills.
- Master’s in Social Work.
- Current Massachusetts license as LICSW.
- Certification in Case Management preferred.
Exposure to infectious/contagious diseases, vision requirements, safety equipment use; tasks typically require 25-50% effort.
