Monday 8am-12pm, Wednesday 8am-12pm, Thursday 8am-12pm and 11am-7pm

SUMMARY:
The Direct Care Professional – Individual Supports (IS) implements individualized programs as specified in Individual Service Plans, provides for individuals’ health and welfare, and ensures general cleanliness and safety in individuals’ homes.
- Job duties may include, but are not limited to, the following functions:
- Instruct and assist individuals with personal hygiene, housekeeping, social skills, advocacy, use of leisure time, and response to emergency situations.
- Facilitate community integration by encouraging individual preference and providing opportunities for recreational activities.
- Encourage and assist individuals to pursue relationships and develop rapport with local community resources, such as banks, physicians, stores, etc.
- Instruct, assist, and teach individuals meal planning, meal preparation, and grocery shopping, while encouraging the individuals’ participation to the fullest extent.
- Monitor individuals’ medical health, assist in making doctor appointments, provide transportation as needed, and advocate for individuals when necessary.
- Address immediate problems in the home or community; notify the direct supervisor or the on-call supervisor.
- Assist individuals with financial management to the degree specified in the Individual Service Plan; and manage petty cash, including documenting and providing receipts for all money spent.
- Instruct and physically assist individuals with housekeeping activities on a daily and weekly basis and promote a sense of pride and ownership in the home.
- When applicable, ensure the home is safe and free of clutter and debris.
- Instruct individuals in fire prevention and evacuation procedures.
- Administer medication and/or monitor medication use for individuals, while documenting in accordance with Department of Public Health policies and procedures.
- Work an assigned schedule that may include weekends and/or holidays.
- Comply with reassignments and directions from the division’s supervisor or director.
- Work modified schedule when individuals’ day programs are closed (site specific).
- Participate in all staff meetings and required training sessions, including in-service training, as scheduled.
- Facilitate house meetings with individuals
- May be required to use employee’s personal vehicle for work purposes.
- Perform other duties as assigned.

- Implement and document individual behavior programs where applicable
- Obtain mandatory training/certification as directed.
- Complete and maintain all required records.
- Provide individuals with transportation as needed, primarily using personal vehicle; however, an Agency vehicle may be available.
- Assist and/or support individuals in emergency situations.
- Implement crisis management as needed and implement PBS consistently
- High school diploma/Hi-SET preferred.
- Skill obtained through Human Rights, Fire Safety, First Aid/CPR, MAP, and site-specific training.
- Ability to assist and/or support individuals as needed and in dead-weight emergencies.
