DAY HOURS! NO WEEKENDS! NO HOLIDAYS! NO NIGHTS!

SUMMARY

The role of the Licensed Practical Nurse is to work under the supervision of the Registered Nurse and to be responsible for the health care services of the individuals supported in Adult Day Services programs.

Essential Functions
  • Administer medications and treatments prescribed by the individual’s Primary Care Physician (PCP) during the time the individual is at the program.
  • Coordinate each individual’s care with other health care professionals, including where the individual resides.
  • Monitor each individual’s health status, and document those findings in their record.
  • As delegated, document any medical conditions or findings in the individual’s record every six months.
  • Report changes in the individual’s condition to the RN; or, in the RN’s absence, report changes to the Program Director and, as delegated, the PCP.
  • Provide oversight of the implementation of the PCP’s recommendations, and, as delegated, provide oversight of the interdisciplinary team’s therapy treatment.
  • As delegated, provide training to staff members on individuals’ medical protocols.
  • Perform other duties as assigned by the supervisor, division management team member, or division director.
  • Participate in all staff meetings and required training sessions, including in-service training, as scheduled.
  • Maintain all program and Agency required trainings.
Qualifications
  • Attainment of a license for a Massachusetts licensed practical nurse
  • Ability to maintain on file a current copy of all required professional licenses, CPR, first aid, and health records
  • Knowledge and skill obtained through at least two years of experience in human services
  • Skill in communicating and working both independently and within a team
  • A qualifying criminal background check, driving record, and a valid driver’s license
